After School Program Rules
PLEASE READ THESE RULLES CAREFULLY AND SIGN ON BOTTOM OF

- Due to permit
regulations, the finish time for the program is 6 p.m. sharp!!
- Late fee is $1 per
minute after 6:05p.m.
- Please
make all checks payable to: Central Queens YM&YWHA
- Personal
checks, money orders and credit/debit cards are accepted. Please see
payment sheet for more details.
- In
all cases of withdrawal, tuition fees shall be paid to the end of the withdrawal
month in which the removal takes place. It is understood that no fee
deductions are to be made, or credits allowed for absence or withdrawal on
account of illness or any cause whatsoever, except as herein above stated.
- All
applications will include a non-refundable one time only $100 combined
registration/snack fee. This is applied only once per family.
- Program
changes may be requested in writing or email.
- Communications
with classroom teachers regarding pick-up changes are the parentfs
responsibility. You must notify your classroom teacher in writing of
any changes in pick-up time along with the after school program either by
emailing Cramirez@centralqueensy.org or by calling (718) 268-5011 x302.
- No
children under the age of 12 are permitted to leave the program without a
parent or guardian.
- All
children participating in classes taking place in the gym or auditorium
must wear sneakers to participate.
- There
is no nurse on the premises during the ASP Hours
- Failure
to comply with the rules may result in removal from the program and
forfeiture of all fees.
Parents:
Please acknowledge your acceptance of these rules by signing and dating
below. Registrations will not be processed unless this page is returned
signed and dated.
